Appalachian State University has arranged a monthly payroll deduction plan for University employees who wish to contribute to the Appalachian State University Foundation, Inc. in this manner. If you wish to contribute to the Appalachian State University Foundation, Inc. through monthly payroll deduction, please print and complete the form below and return to: Advancement Services, 309 Founders Hall.
Contributions to the Appalachian State University Foundation, Inc. are tax-deductible, to the extent allowable by law.
